The way that Dr. Stephanie Daniel works with patients differs considerably from the conventional medical model you may be accustomed to. Please read the following information thoroughly to familiarize yourself with the process.

The Patient Journey

Dr. Daniel’s case review process is unlike that, which any other doctor uses, and rather, much more resembles the work of a detective who carefully analyzes all pertinent info, determines what questions arise from the information at hand, reviews what is available thus far both as pertinent findings and pertinent negatives (what’s already been ruled out), then sets out to acquire the outstanding pieces of evidence needed to make an accurate diagnosis and develop a plan for recovery. After the full case review is complete, the diagnosis will be identified as well as the full overview of how to address all of the layers involved in a step by step, methodical and systematic process. This is then further broken down into phases of actionable steps, with the expectation that customizing the plan further will need to take place in subsequent follow-up appointments along the way that are indicated by monitoring and observing the healing that unfolds in a dynamic process.

Functional Medicine SF takes our role as your partner in achieving your health goals very seriously, and therefore wants to ensure this is the best place for you to do that. To get started, we begin with having every new patient do a 15-20 minute video consult with Dr. Daniel. This is to create an initial connection to ensure a good fit and to give you the opportunity to know that Functional Medicine SF is the right place for you. It it our role as your guides, to help create a container of support for you, while honoring the commitment to hard work and accountability to yourself throughout your transformational journey to health.
Upon completion of this video consult, should we decide to move forward together, you will have the opportunity to start your case review process by completing your health journey questionnaire, complete a medical symptom questionnaire, receive an order for initial blood work (from the best lab based on your insurance) and set up the initial full intake appointment.
The 20 minute video consult also allows Dr. Daniel to assess who you may best work with, whether it be with her, one of the FMSF Physician’s assistants or everyone in a team based approach to care.

Scheduling an appointment; Initial appointments are made via the patient portal in MD-HQ Once you have registered, send up a secure message in our online medical record system (MD-HQ: Patient Portal) that you are ready to schedule your New Patient appointment. You are required to complete your intake forms prior to scheduling. Please upload any prior lab results, supplements and medications you may be taking to your "Patient Portal" in MD-HQ. Established Patient: request an appointment time via the Patient Portal*
Continuing Care ; To ensure that each patient understands their commitment and expectations and to increase our quality of care we would like to point out the following: Follow-up consultations must take place per patient recommendations. Continual failure to comply with protocol recommendation timeline will result in dismissal from functional medicine SF.*
You are REQUIRED to have a separate primary care doctor; VERY IMPORTANT Dr. Stephanie Daniel offers functional medicine consultation only. We do not offer after-hours or urgent care – you must have a primary care doctor for this purpose. This also means she is unable to respond to urgent requests of any sort, including messages and phone calls.*
